Incident Narrative
On 7/31/2017, an employee was trying to repair a spiker machine. The employee was working under the machine and could not reach a broken screw. Another employee got in the machine and made an adjustment to the anvil so that the other employee could reach the screw. After the adjustment was made, the employee accidentally pumped the controls for the anvil again, causing it to cycle and pin the other employee's right hand to the track. The anvil then hammered approximately 3 to 5 times, crushing the employee's right index finger and requiring surgical amputation. The machine was not locked/tagged out at the time.
