Incident Narrative

A forklift operator attached a chain around a skid, but the tension was too tight in the chain. The injured employee was checking on the tension when the forklift operator got off the forklift, allowing it to roll forward. The employee's hand was then caught between the skid and chain, amputating the right index finger.
