Incident Narrative
An employee was climbing an 8-foot ladder at the gallon milk filling machine to take a quality sample when the ladder shifted. The employee fell and her leg was caught between two rungs of the ladder, causing her to swing downward and hit her head on the floor. She suffered a head contusion, a laceration above the right eye, right wrist and elbow contusions, a broken ankle, and a broken toe on the left foot.
