Incident Narrative

A temporary employee was operating a horizontal bandsaw. The employee started to adjust the in-feed rail on the saw using a wrench while the saw was running. The wrench slipped in the employee's hand and his right index finger contacted the saw blade. The employee was taken to the hospital and the index finger was amputated in the hospital the day after the incident.
