Incident Narrative
On 5/22/2017, at approximately 10:30, an employee was installing an auger inside the housing of a larry car. The employee rigged the auger with chain falls and come-a-longs to position it inside the housing. During the installation, the employee's left index and middle fingers were caught and partially amputated between the auger and the housing. The equipment was locked out and not powered at the time of the incident.
