Incident Narrative

Two employees were working from a column form wall about 19 feet in height to complete rebar tie-in on the cage of the column. Both employees were descending from the column form wall when they lost their footing and fell to the concrete slab below. One employee sustained a crushed heel and broken lower leg and was hospitalized. The second employee sustained a back injury.
