Incident Narrative

An employee was securing a cabinet to the wall. He had one foot on a ladder and the other on a shelf so he could reach the top of the new cabinet. The shelf anchor failed, causing the cabinet and employee to fall approximately 42 inches. He suffered contusions to the right flank, a right rib fracture, and hip pain and needed right arm stitches.
