Incident Narrative

On Saturday, May 13, 2017, at approximately 10:45 AM, an employee was slicing meat using a meat slicer. She placed her right hand on the slicer hold-down knob but her right thumb moved to the side of the hold-down/guard. As she proceeded to slice the lunch meat, her right thumb contacted the blade, amputating the thumb tip.
