Incident Narrative

An employee was using a string trimmer while standing on an uphill incline when he lost footing as a mower approached from behind. His foot slid backwards and into the path of the mower deck, which severed his Achilles tendon and ligaments and broke bones in his lower leg. A mulching block-off plate was not in place on the mower's discharge opening and pulley guards were not installed at the time of the incident.
