An employee was working on the second level of a scaffold approximately eight feet high. The employee was reaching up to adjust the toe board for the level above when he slipped and fell through the mid-rail to the ground below. The employee suffered a left shoulder and elbow injury and a laceration above the left eye requiring stitches.
