Incident Narrative
An employee was installing a mixer with a coworker. His right hand was inside the pulley assembly, attempting to get the cable back on the pulley, when the up/down air lifter was actuated to relieve tension on the cable. The mixer then came down and amputated his right index fingertip between the knuckle and the nail.
