Incident Narrative

An employee was cleaning a sewer drain with a high-pressure hose. The hose had to make a 90 degree turn at the bottom of the manhole. Instead of making that turn, the hose circled back around and came out of the manhole, striking the employee on the left arm and causing three lacerations.
