Incident Narrative
An employee was wiring a terminal block inside a breaker control cabinet. The employee repositioned himself inside the cabinet and struck his head on a bolt. After striking his head, the employee fell over and was shocked by a 240 volt AC knife switch resulting in a burn to the back of the neck, a head laceration and an electrical burn to the right thumb and index finger. The employee also sustained right arm numbness.
