Incident Narrative

On April 10, 2017, at approximately 11:15 a.m., an employee was performing maintenance on a rooftop HVAC unit when he fell 19 feet through a skylight to the ground. A personal fall arrest system was not worn at the time of the incident. The employee was hospitalized with severe internal injuries such as a lacerated liver, punctured lung and head trauma.
