Incident Narrative
An employee was inspecting a guide box outside of the millhouse when he attempted to pull a 20-pound pin on the guide box. His left hand was then caught and crushed between the pin handle and the box, and his left index, middle, and ring fingers had to be surgically amputated above the first knuckle.
