Incident Narrative
An employee was replacing switches on the outriggers of a digger derrick. After the employee replaced the switches he adjusted the outriggers. The employee placed a bolt in the access hole of the outrigger to adjust it and pulled the lever to raise it. As the outrigger came up, it caught his right middle finger between it and the bolt, resulting in a fingertip amputation.
