Incident Narrative
An employee was working from a motorized order selector disassembling racking. A support beam fell striking the employee working from the order selector. The employee then fell from the order selector to the concrete floor resulting in broken bones. The employee was wearing a body harness connected to the order selector, but still made contact with the floor.
