Incident Narrative
An employee was providing assistance to a customer while using a caustic tank. The customer product inlet line had no shutoff valve and the product hose had a downward angle to the truck tank unloading pad. When the employee blew down the product hose, he was sprayed in the face by caustic sodium hydroxide solution, resulting in burns to his mouth, lips, and face.
