Incident Narrative

An employee was unloading freight from a silage trailer at a customer location. The silage trailer had a moving floor inside that runs on a web conveyor belt. The employee opened the door in front of the trailer when the web conveyor (PTO) was operational and got caught in the conveyor. The employee's left arm was amputated.
