Incident Narrative

An employee and a coworker were changing out an actuator cylinder. They were using supplied air to operate the cylinder and align the holding pin by lowering and raising it. The employee aligning the holding pin asked the air supply operator to put air into the line. Afterwards, the employee was checking the alignment when the cylinder dropped and amputated the employee's left middle fingertip before the bone.
