Incident Narrative

An employee and a coworker attempted to re-activate a dock plate after servicing the hydraulic pump. The employees removed their lockout devices and found that the dock plate would not lower. The injured employee went underneath the dock plate to attempt to adjust it to lower properly. As he was in the process of doing this task, the dock plate fell onto his body, puncturing his lung, breaking ribs, and breaking his left scapula.
