Incident Narrative
An employee was using a dolly to transport boxes. When she pulled back on the handle, she was unable to keep it upright and the dolly came down, trapping her finger between the handle and ground. When she attempted to pull her finger out, her skin was removed and her fingertip had to be surgically amputated.
