Incident Narrative
On March 11, 2017, an employee was operating a forklift moving a bundle of square tubing off the rack in the yard. The employee walked underneath the load on the forklift when the operator told him to move because there was a piece of metal sticking out. The employee attempted to move when the piece of metal fell to the ground, bounced up at an angle, and struck the employee on the back of his left leg, breaking it. His leg had to be surgically amputated.
