Incident Narrative

The employee was assembling a pneumatic activated butterfly valve assembly. He also attached the air source to it. As the employee was reinstalling the valve his right thumb was inside the valve assembly. The valve activated pinching the employee's right thumb tip resulting in amputation of the thumb tip including bone and the nail.
