Incident Narrative
An employee was loading a pair of forks for craning onto a delivery truck. The pair of forks have a sliding mechanism that helps with loading and unloading. One of the forks slid towards the employee as he was loading the truck and his right middle fingertip was caught and amputated (no bone).
