Incident Narrative
An employee was sweeping the area in front of a work site when a flatbed truck containing post tension cables arrived. The truck pulled in through a gate and stopped to begin off-loading the post tension cables. The forklift operator unlatched the post tension cables and then noticed that the truck had stopped near a mound of dirt which the operator could not access with the forklift. When the truck moved forward, a post tension cable shifted and fell off the truck striking the employee who was sweeping in the face causing multiple lacerations and severing the top part of his left ear.
