Incident Narrative

An employee was driving a truck to a farm to deliver feed. As he turned onto the farm's driveway, the left rear wheels of the truck trailer ran off the driveway's culvert, causing the truck to turn over on the driver's side. He suffered three broken ribs and a lacerated spleen.
