Incident Narrative

An employee was beginning to do a pre-operational inspection on equipment. She was visually inspecting a running tail cutter when she noticed some debris in the wheel. She put on her glove and attempted to clear the machine of debris. When she did, her glove was caught by the equipment, which then amputated her right index fingertip.
