Incident Narrative
An employee was changing out a motor on a drag conveyor. Lock out/tag out was removed and power restored to check rotation. When the motor started, the employee's fingers were pulled into the chain and gear box amputating the left index and middle fingertips. The left ring finger sustained a laceration requiring stitches.
