An employee was using a pipe threading machine to attach a four-way coupling onto a threaded steel pipe. The employee used a pipe wrench to hold the coupling in place. When the pipe wrench was released from the coupling, the foot pedal was engaged and the pipe wrench came down and crushed the employee's fingertip, amputating it.
