Incident Narrative
An employee was moving pallets with a forklift when it became stuck in the mud. Two coworkers attempted to pull the forklift out of the mud by tying two straps of fastener together with a clevis and attaching it to a truck that was pulling the forklift. The clevis then broke and swung back, striking and lacerating the employee's head.
