An employee was working on an overhead garage door. He was in the bucket of a fork truck when he lost his balance and shifted his weight to the right side. The bucket's screws popped out and the bucket came apart. The employee fell 10 feet to the concrete floor, cracking his ribs and puncturing his lung. He was hospitalized.
