Incident Narrative

An employee was waiting inside of his vehicle for the pipe to pull through and heard the equipment make an erratic sound. The employee stepped out of his vehicle to check on the pulling equipment. As he approached the cable pulling equipment, the cable released from its guide/pulley and whipped back and forth striking the employee and breaking an arm.
