Incident Narrative

An employee was rewinding the label on a label machine using her left hand. She used her right hand to jog the machine forward and was using her left hand to load the paper on the paper winder. The tip of her glove was caught causing her left hand to wrap around the rotating post and the cardboard core resulting in a left wrist fracture and dislocation as well as a fifth index compound fracture and a fourth index fracture.
