Incident Narrative

An extrusion machine became backed up. Employees dissipated the energy and were going to remove the machine's die plate when a pressure buildup caused the plate to blow off and strike an employee in the right arm. He fell onto his left side, fracturing three ribs and separating his right shoulder.
