Incident Narrative

An employee was helping another employee lower a walk-behind pallet jack that had been stood up on it's back end. The employee's right hand had a hold of the forks and his right index finger got stuck in the wheel well as it was lowered to the ground. The finger was smashed and the tip was amputated at the hospital.
