Incident Narrative

An employee was cleaning out a gate valve while on a scissor lift about 15 feet in the air. As the employee opened the inspection door on the valve, hot ash dislodged and landed on the lift's control panel. The hot ash on the control panel made it impossible to lower the lift. The employee manually lowered himself from the lift and fell on his back.
