Incident Narrative

An employee was performing forklift/rigging operations (material handling) with a telescopic handler (telescoping forklift). While adjusting the forks on the handler, the employee's right index finger was caught in a pinch point between the adjustment box and the forks, lacerating and fracturing it. The finger was amputated down to the first knuckle.
