Incident Narrative

An employee was trying to remove packages from a chute and was unable to remove one due to a closed dock door. The employee then opened a loading bay door believing a trailer was in the bay to be entered. The employee stepped off the dock bay surface and fell 46 inches, suffering a punctured lung and pelvic and rib fractures.
