Incident Narrative

An employee was working on the chain and sprocket to a tape machine. As he used a tool to tighten the chain, his hand slipped and his right index finger contacted the moving chain. His right index finger was partially amputated (approximately 2-2.5 cm). The machine was running and was not locked/tagged out at the time of the incident.
