Incident Narrative

An employee on a ladder was reaching to remove some clothes from the back of a rack while holding on to a horizontal pipe (used to hang clothes) near the front of the rack. The horizontal pipe disengaged from the rack uprights and fell, causing the employee to lose her balance and fall to the ground. She fractured her left ankle.
