Incident Narrative
On December 4, 2016, an employee fell from a portable 4-foot, fiberglass A-frame ladder that had been set up on the finished floor and was covered with paper. The floor vents had not been installed and one of the ladder legs was on the paper that covered a vent floor opening. The injured employee had climbed the ladder to approximately the second or third step when the ladder tipped as the leg dropped into the floor hole. He fell and his lower body struck the floor while his upper torso struck against the wall corner. He suffered two fractured right ribs and a punctured right lung, requiring hospitalization.
