Incident Narrative

An employee was drilling bolt holes to fit the scrape blade on a snow plow. As the employee attempted to clear a metal shaving out from behind the bit, the employee's sleeve was caught and was pulled into the drill chuck, suffering a broken forearm. The drill chuck was guarded at the time.
