Incident Narrative

An employee was operating a sit-down forklift to stack large bundles of flat corrugated boxes in rows. A row of bundles started to fall in a domino fashion, striking and then tipping the forklift over from the employee's right to left side. His left foot was pinned under the forklift and several bundles of boxes fell on top of him. He was hospitalized with a fractured left foot.
