Incident Narrative

An employee was on a 5-foot ladder cutting a pipe used to carry nitrogen. As the employee was cutting the pipeline, the pipe (which was filled with carbon dioxide) ruptured. The employee suffered a concussion, fractured orbital bones, a skull fracture, and neck injury.
