Incident Narrative

An employee was transforming hot resin from a 55-gallon drum that was elevated by a forklift over an open vat. The employee opened a gate valve on the drum and the valve popped off, creating an uncontrolled release of the hot liquid that caused serious burns to his right forearm. He was hospitalized.
