Incident Narrative
An employee was installing a pin in a boom point that had been chilled in liquid oxygen. When the employee removed the pin with a slide hammer, an ignition occurred and caused the pin to shoot out of the hole and to strike the employee. The employee suffered burns to both hands and a broken thumb.
