Incident Narrative

An employee was packaging lunch meat containers on a conveyor line when one of the container lids became dislodged. When she tried to retrieve the lid, her fingers were caught in the middle of the two adjoining conveyer lines. Her right ring and middle fingers were severely lacerated, and her ring finger had to be medically amputated.
