Incident Narrative

An employee was replacing roller bearing adapters on a railcar when the jack moved, causing the side frame to move. When the jack moved, the employee's right hand was partially between the adapter and the side frame, compressing his right middle and ring fingers and amputating the first joint of his index finger.
