Incident Narrative

An employee was attempting to check the valve position on top of a small bulk tank. He placed a ladder against the tank to get a better view of the valve and was climbing up. As he reached the half way point, the ladder slipped on the smooth concrete surface and he fell 4 to 6 feet and landed on his right knee, breaking his kneecap.
